If abroad, loss of passport should be immediately reported to the nearest Police Station and to the Passport Office, or India for NRI.

  • Photocopy of the old passport is not a mandatory document to be submitted in case of lost/damaged/stolen passport. It should be produced if available.
  • However, you will need to provide previous passport details like passport number, date of issue, date of expiry, and place of issue while filling the passport application form.

  • In case of a lost passport, you need to apply for re-issue of passport.
  • Required documents include: Proof of Present Address, Proof of Date of Birth, Affidavit stating how and where the passport got lost/damaged (Annexure 'F'), Police report in original, Self-attested photocopy of first two and last two pages, including ECR/Non-ECR page of old passport (if available), and Original EC/Seizure Memo.

Applicants less than 18 years of age are considered as minors for issuance of passport.

  • Endorsement of a child's name in a parent's passport is no longer allowed.
  • As per the amended instructions, a minor should have a separate passport.

  • Ideally, both parents/legal guardians should sign Annexure ‘D’ in fresh or re-issue cases.
  • Both parents should be present at the PSK/POPSK while applying, or one parent with passports of both parents may be present.
  • If a parent is abroad, the local parent will sign and obtain the other parent’s consent.
  • If consent is not possible, provide a declaration in form of Annexure-C with applicable reason.

  • Children between the ages of 15 and 18 have the option to apply for either a minor passport valid until they turn 18 or a full validity passport of 10 years.
  • Different fees apply for different validity periods.

In such cases, minors can be issued a passport under the Tatkaal scheme subject to the approval of the Competent Authority.

  • Senior citizens (age 60 and above) receive a 10% discount on the applicable passport fee for all fresh applications.
  • They also receive assistance at PSK/POPSK with special tokens for expedited processing.

  • On the date of appointment, applicants should visit the PSK/POPSK nearest to their hostel/institute address.
  • Mentioning the parental address in the permanent address column of the application form is mandatory.

  • Government/Public Sector/Statutory body employees must submit either Annexure 'A' (Identity Certificate), Annexure 'G' (No Objection Certificate), or Annexure 'H' (Prior Intimation Letter) as additional documents.

  • By Birth: Citizen of India by birth if born in India, subject to provisions in The Citizenship Act, 1955.

To endorse the spouse's name on your passport, you have to apply for a 'Re-issue' of passport through India for NRI.

  • While mentioning the name of a foreign spouse in the passport application form, no prefix/suffix denoting nationality should be added.
  • Only the actual name of the spouse needs to be mentioned.

No additional document is required for deletion of the spouse's name on account of divorce/separation.
